Non-adherence is a very common experience, and a significant public health concern that prevents patients from realising the full benefits of their treatment, and negatively impacts on individuals and their families. If you are 18 years or older, AND, are prescribed an antidepressant medication, please take 5 minutes to complete a very easy questionnaire about your medication. By taking part in this study you can help us to understand why some people stop taking their medication, and to develop better ways of helping those who have difficulty sticking to their medication. Participation is completely voluntary, confidential and anonymous, so no personal information will be provided to anyone at all. We also encourage you to share this with other family members, friends or colleagues who are eligible to take part in the survey, to help us reach our target. This study has been approved by the South African Depression and Anxiety Group (SADAG) and the University of South Africa (UNISA). The results of the study will be published in early 2015.

To complete the questionnaire, choose one of the options below that is most convenient for you:

1.    ONLINE: Click here for the link to go to Survey Monkey and fill in the questionnaire online. www.surveymonkey.com/s/SADAGAdherenceSurvey

2.    Download here, complete it and e-mail it back to This email address is being protected from spambots. You need JavaScript enabled to view it. OR fax it back to 011 234 8182,

3.    To complete the survey over the telephone, you can SMS “Adherence” to 31393, or PHONE 0800 33 33 77, and we will call you back.
